Suppliers wishing to participate in South Dakota Medicaid should visit a supplier. Exisiting suppliers should also visit the supplier`s maintenance and revalidation. Non-use that is not permitted is prohibited, including to illustrate and not limit, copies of CPT for resale and/or license, transfer of copies of CPT to a party unrelated to this agreement, the completion of modified or derivative work of CPT or the commercial use of CPT. The license for the use of CPT for any unauthorized use must be obtained through WADA, CPT Intellectual Property Services, 515 North State Street, Chicago, IL 60610. Applications are available on WADA`s website www.ama-assn.org/go/cptExternal_Link. SD Medicaid uses an online registration application process with the necessary support documents sent by email. Applications must be completed and supporting documents must be received within 30 days of the start date of the application, or the application is rejected/rejected.
